You can locate your provider or practice by entering your practice’s unique code. The unique code can be found on your practice’s Patient Portal website. The same code is also located in the feeder section of any e-mails your practice has sent to you in the past. If you’re using healow App, enter the practice code from any of the screens below: If you’re using healow.com, enter the practice code in the Search by Provider Name OR Practice Name/Code field. 1. Click the Access your Health Record button: 2. Locate your practice by typing in the practice name OR the practice code. The practice code is a unique, six-letter code that identifies the practice, and it can be found on the practice’s Patient Portal website. 3. Type in the city/state and then click Go. 4. Select your practice from the list. If your practice is not on the list, please reach out to your practice directly and ask them to join the listing! 5. Enter your username and password: Note: Usernames and passwords are case sensitive so be sure you enter your credentials correctly. 6. Validate your identity by entering your date of birth OR phone number, and then click Next: After validation, you must create a new password to replace the temporary password. 7. Enter your new password, enter it a second time to confirm it: 8. Select your security question and answer, click Agree: 9. To view a another family member’s health information in the Circle of Care, it is necessary to link your account to the other person’s account. This linking process will require you to log in as that person to view their records. 1. Click Add Family Member: 2. Select the family member’s account and enter that person’s username and password: 3. Check the box indicating that you have read the consent form and then click Agree: 4. Click the Link Account button: Your portal account has now been linked.
